Louise and Wilbur Cook had no children. Wilbur Cook was the doctor who signed the death certificate of his mother-in-law, Amelia Higginson Thurtell, in 1917, signing his name as Dr. W. D. Cook, 59 East Madison Street, Chicago, Illinois, which was probably his office address since that is in the Loop in downtown Chicago.

The death certificate for Wilbur Daniel Cook shows he lived at 4636 Hazel Avenue, Chicago, Illinois. His wife, Louisa Cook, was the informant for the death certificate. The first name of his father is illegible - it looks like Fseral Cook. The maiden name of his mother is also hard to read - it looks like his mother's name was Mary E. Burnell.
