Notes for Wilbur Daniel COOK

The family tree done by Harriet Thurtell about 1900, the 1963 Thurtell Family History, and the History of the Thurtell Family done by Susan Persia Thurtell about 1968 show Wilbur Cook was an eye specialist in Chicago, Illinois. Louise and Wilbur Cook had no children. Wilbur Cook was the doctor who signed the death certificate of his mother-in-law, Amelia Higginson Thurtell, in 1917, signing his name as Dr. W. D. Cook, 59 East Madison Street, Chicago, Illinois, which was probably his office address since that is in the Loop in downtown Chicago.

The death certificate for Wilbur Daniel Cook shows he lived at 4636 Hazel Avenue, Chicago, Illinois. His wife, Louisa Cook, was the informant for the death certificate. The first name of his father is illegible - it looks like Fseral Cook. The maiden name of his mother is also hard to read - it looks like his mother's name was Mary E. Burnell.

According to the Chicago Daily News of August 24, 1920, burial was at Memorial Park, Evanston, Illinois. The obituary in the August 24, 1920, Chicago Tribune states that he was attending physician at John Worthy school for many years and that his friends said he never refused a charity case. It shows that he died at his home at 4626 Hazel Avenue, Chicago, and was a well known occulist. His office address was 5 South Wabash Avenue, Chicago, Illinois. He was 59 at the time of his death, which was due to a cerebral hemorrhage according to the death certificate, with a secondary cause arteriosclerosis for the prior ten years.
